If you volunteered or were at all involved in the oil spill cleanup last December, the San Francisco Department of Emergency Management wants to hear from you. They've released a short questionnaire (in MS Word format) that you can fill out and return either by email or fax. SF DEM are looking for any and all feedback from the people "on the ground" about how the procedures worked (or didn't work) to hopefully continue improving their response system.
